Your Hormonal Symptoms Are Not A Hormone Problem

Here's a question worth sitting with:
If your hormone levels test normal what is causing your hormonal symptoms?
Because the symptoms are real. The cyclical bloating that arrives on schedule. The mood that shifts like a switch was flipped. The skin that breaks out in the same spots every month. These are measurable experiences in a real body. And yet: normal tests.
Here's what those tests are not measuring.
The clearance problem
Your liver clears oestrogen from your body every single day. It's not a monthly event it's continuous. Every day, used oestrogen goes through two phases of detoxification in the liver, gets packaged up, and exits via bile into your gut and eventually out of your body.

That process has three stages:
Phase I breaks oestrogen down into metabolites. Phase II packages those metabolites for safe removal attaching molecules like glucuronic acid to make them water-soluble. Then those packaged metabolites exit through bile, into the gut, and out.
When oestrogen metabolism is inefficient, it can lead to a build-up of more potent oestrogen byproducts or a re-circulation of oestrogens that were meant to be eliminated. This can contribute to a state of oestrogen dominance — where oestrogen levels are functionally higher relative to progesterone — worsening symptoms like breast tenderness, bloating, heavy periods and PMS.
Your blood test measures circulating oestrogen levels. It does not measure how efficiently your liver is clearing it. It does not measure bile quality or flow. It does not measure whether the oestrogen your body already used is being properly excreted or recirculating.
A normal test tells you your body is producing a normal amount of oestrogen.
It tells you nothing about what happens to it after.
Where bile comes in
This is the piece that connects directly to everything else I write about in this newsletter.

Bile is the vehicle that carries processed oestrogen metabolites out of the liver and into the gut for final excretion. When bile flow is sluggish when bile is thick, insufficient, or poorly stimulated oestrogen that was successfully processed in Phase I and Phase II has nowhere to go efficiently.
It recirculates.
And when it recirculates, it binds to oestrogen receptors again. Even in women whose ovaries are producing a completely normal amount of oestrogen, poor clearance creates a state of functional excess.
That's the PMS that gets worse every year even though "nothing has changed hormonally." That's the cyclical bloating that arrives like clockwork in the luteal phase. That's the skin that breaks out on schedule. The mood. The sleep. The breast tenderness.
Not too much oestrogen being produced. Oestrogen that was produced, used, and was supposed to leave but didn't fully clear.
The gut completes the picture
There's a third phase that most hormone content never mentions.

After the liver does its work, oestrogen metabolites enter the gut via bile. Here, specific gut bacteria the "estrobolome" are responsible for the final clearance.
